Clinical Nurse Consultant – Integrated Chronic Care for Aboriginal People Program – Targeted Location: Wonnarua Country – Cessnock District Hospital Employment type: Permanent Part Time – 16 hours per week Remuneration: $68.50–$69.87 per hour + super + salary packaging Applications close: Sunday 25th January 2026 Job Summary We are seeking an Aboriginal Health Registered Nurse to provide clinical expertise, case management and coordinated chronic disease care for Aboriginal people across Cessnock, Muswellbrook, Singleton and Maitland. The role combines community outreach, care planning, GP liaison and service linkage. Responsibilities Conduct clinical assessments and develop care plans for Aboriginal patients Manage case coordination and follow‑up to ensure continuity of care Collaborate with local GPs, primary health networks and community services Provide culturally safe education and support to patients and families Attend team meetings and contribute to program improvement Document care activities in accordance with health system standards Qualifications & Experience Registered Nurse with current authority to practice (AHPRA) Minimum 5 years full‑time equivalent experience post‑registration, including at least 3 years in chronic disease nursing Approved postgraduate qualifications in chronic disease management Proven experience in culturally safe care for Aboriginal and Torres Strait Islander communities Strong communication skills and ability to work within a multidisciplinary team Commitment to ongoing professional development We Support You Structured orientation and mentoring Weekly education sessions on Monday Regular ICCAPP team meetings Training in chronic disease and culturally safe care Access to the CNC network for continuous learning What We Offer Monthly allocated days off (full‑time) 6 weeks annual leave for eligible full‑time nurses Paid parental leave (eligible employees) Salary packaging up to $11,600 + novated leasing Fitness Passport Employee Assistance Program Opportunities for leadership and progression in Aboriginal health Special Features of This Role A unique opportunity to influence Aboriginal health outcomes with a culturally respectful, community‑driven approach within a supportive team environment.
