TheRole As a therapist at Connect2Care, you will be joining amultidisciplinary team. You will provide evidence-based services to peoplethrough multiple service delivery models, including telehealth, communityvisits and centre-based services. Referrals may come through the NDIS, Medicareor privately. It is a mobile role, with the option to work from home or ourhubs when not seeing clients. The role will involve the following: Assessing and providing interventions forchildren & adults living with a range of disabilities Working within the NDIS to maximise clients'level of function, capacity & social engagement Assisting clients with assistive technologyassessment, recommendations and applications (in relevant fields) Timely documentation & report writing Managing a varied caseload Meeting company professional expectations (e.g.meeting billables & effective client management) Building relationships with key stakeholders,including therapists, clients, & referral sources Working across leading software platforms &online tools The NewGrad Program Connect2Care has developed and specifically tailored a NewGraduate Program to make the transition smoother for new graduates entering theworkplace. The program fully supports recent graduates, enabling them toacquire the skills needed to launch them into a successful career. Spanningover 12 months and starting from the moment you step through our doors.
