We are looking for an experienced Dietitian to join our team as a full‑time Program Dietitian. In this role, you will deliver the Medibank Type 2 Diabetes and Better Knee Better Me programs to eligible participants, supporting their weight loss and chronic disease management. This is a work‑from‑home position, where you will manage a caseload of participants through digital program delivery using phone and video platforms. Using your clinical expertise and a strong understanding of health behaviour change methodology, you will empower participants to make positive changes to their health and wellbeing, helping them better manage their condition and achieve their weight loss goals. Key responsibilities in the role include, but are not limited to: Conducting initial health assessments and develop tailored participant weight management plans aligned with program methodology and evidence‑based best practices. Scheduling and delivering follow‑up consultations via telephone or video, ensuring a person‑centred and seamless weight management support program. Performing health and risk assessments, including basic clinical measurements (e.g., height, weight, BMI) and collect quality‑of‑life data, supporting participants in self‑reporting and self‑monitoring. Integrating health behaviour change principles into program delivery and provide regular health coaching sessions to help participants achieve weight loss goals and/or manage chronic conditions. Supporting program evaluations to demonstrate clinical, operational, and financial outcomes. Building and maintaining strong relationships with stakeholders to ensure effective program delivery. About You You are passionate about preventative health and have a natural ability to build rapport and communicate effectively with a diverse range of participants and healthcare professionals. In this highly autonomous role, you bring an organised and flexible mindset, enabling you to meet the unique needs of your clients while delivering exceptional care. You are a Dietitians Australia registered Accredited Practising Dietitian with: Experience delivering health coaching and providing seamless, effective dietetic support to people with type 2 diabetes, knee osteoarthritis, and other chronic illnesses. 5+ years’ experience in chronic disease management as a dietitian. Expertise in weight management programs, particularly using Very Low Energy Diets and meal replacements. Proven ability to deliver programs remotely via phone or video. A solid understanding of health behaviour change methodology and its practical application. As this is a work-from-home role, you can be located anywhere in Australia and you will need a suitable home office area and sufficient speed and reliable internet connection (50Mpbs min download speed required).
