Social worker Grade 1 or 2 – ABI Unit – Alfred Health Alfred Health is a leader in health care delivery, research, and education, serving the inner‑southeast suburbs of Melbourne. In 2026 it will merge with four regional health services to become Bayside Health, a merged entity supporting more than 1.2 million Victorians. Position Summary This role supports patients with Acquired Brain Injury (ABI) by identifying and meeting their complex psychosocial needs. The Social Worker collaborates with clinical units, provides advanced case‑work expertise, and builds community linkages to facilitate smooth transitions from hospital to rehabilitation and community settings. Responsibilities Identify and address the psychosocial impacts of illness and hospitalisation for ABI patients and their families. Collaborate with multidisciplinary clinical teams and external stakeholders (NDIS, TAC, Workcover, ACAS, TCP, Carer Services, OPA, VCAT). Consult with staff and provide advanced case‑work expertise to manage complex clinical cases. Develop and maintain community linkages to support patient transition. Demonstrate flexibility, negotiation, and communication skills and work effectively as part of a team. Experience And Qualifications Required Eligibility for membership of the Australian Association of Social Workers. Bachelor of Social Work or a recognised equivalent. Experience in an acute hospital setting is preferable. Strong initiative, facilitative qualities, and ability to work effectively in a multidisciplinary team. Essential Previous experience working within a hospital setting. Knowledge of NDIS, aged‑care services, and TAC funding models will be advantageous.
