About us Geriatric Care Australia (GCA) is a mission-driven clinical services provider improving outcomes for older adults in residential aged care across Australia. We’re expanding and looking for a full-time Nurse Practitioner (part-time also considered) to join our Sydney metro team. Hybrid flexibility : Mix of on-site residential aged-care with some telehealth WFH Impactful work : Help prevent hospitalisations and improve resident outcomes Growth opportunities : CPD, leadership opportunities, new model‑of‑care pilots Purpose‑led culture : A mission‑first organisation focused on improved outcomes for older people Attractive remuneration : A competitive package with performance bonuses The role Work directly with an in‑house multidisciplinary team of GPs, geriatricians & RNs Lead advanced assessments, diagnosis and management of complex older adult Provide timely intervention for acute deterioration (infections, falls, delirium, etc.) Order investigations, prescribe/de‑prescribe and optimise medication plans Deliver proactive chronic disease management and preventive care Support families, transitions of care and MDT discussions Use digital tools and telehealth to extend access and continuity of care This is not an isolated NP role — you’ll work alongside a multidisciplinary medical team (GPs, geriatricians, RNs) at Geriatric Care Australia. What you bring AHPRA RN + Endorsed Nurse Practitioner with no restrictions Eligible for (or holding) a Medicare provider number Strong experience in aged care, geriatrics, sub‑acute, community or palliative care Confident managing acute and chronic conditions in older adults Excellent prescribing, clinical decision‑making and communication skills Competent with EMRs, telehealth and digital workflows NSW driver’s licence and ability to visit aged‑care homes across Sydney The practical details Location: Sydney metro—primarily Inner West, North Sydney & CBD Model: Hybrid (on‑site + some telehealth from home) Hours: Full‑time or part‑time available Salary: base + super + performance bonuses Start: December/January/February 2026 (flexible) Interested?
