Learning Disability Liaison Nurse

We are seeking a compassionate and experienced Band 6 Learning Disability Liaison Nurse (LDLT) to join our dynamic multidisciplinary team. The post holder will work closely with acute hospital teams across Sussex to ensure that people with a learning disability receive equitable, safe, and effective care during inpatient admissions. This role plays a critical part in implementing the learning from LeDeR reviews and meeting the trust's responsibilities to reduce health inequalities and improve health outcomes for people with a learning disability. The LDLT nurse will act as a specialist advisor, supporting hospital staff to make reasonable adjustments, improving understanding of complex communication needs, and ensuring person-centred care planning. The post holder will advocate for individuals and their families, championing best practice in inclusive health care, and contributing to safeguarding and risk assessments to prevent avoidable harm such as aspiration, falls, and self-injury. This is a varied and rewarding role requiring clinical knowledge, experience in liaison or community learning disability nursing, and strong partnership working with acute services, primary care, community teams, and families. The successful applicant will have a sound understanding of legislation including the Mental Capacity Act, Equality Act, and Care Act, and will demonstrate commitment to co-production and continuous improvement. Main duties of the job The post is 0.6 FTE (22.5 hours per week), offering flexible working and opportunities for development within Sussex Partnership NHS Foundation Trust, an organisation that values diversity, innovation, and wellbeing. To provide specialist liaison support within the acute hospital setting (RSCH), ensuring individuals with a learning disability receive safe, effective, and person-centred care during inpatient admissions or outpatient appointments. To work proactively with hospital staff to identify patients requiring additional support, implementing reasonable adjustments and providing clinical advice around dysphagia, aspiration risk prevention, pain assessment, and sensory needs. To act as a key contact for hospital staff, families, and community learning disability services, ensuring robust communication, discharge planning, and continuity of care. To participate in safeguarding processes, capacity assessments, and clinical risk assessments, particularly where concerns arise around neglect, self-harm, or premature mortality. To maintain accurate clinical records, contribute to audits, and support quality improvement initiatives aligned with LeDeR learning and the NHS Long Term Plan.
