This role is ideal for someone who enjoys working independently, building meaningful relationships, and ensuring clients receive safe, personalised care in their own homes. About the Role You will work closely with our multidisciplinary team to deliver medication reviews, education, and ongoing support for clients living in the community. Your focus will be on promoting safety, enhancing medication understanding, and improving overall health outcomes. Key Responsibilities Conduct in-home medication reviews and reconciliation Provide advice on safe and effective use of medicines Support clients with medication management, including compliance aids Liaise with GPs, nurses, carers, and family members to coordinate care Identify medication risks and propose appropriate interventions Maintain accurate and timely clinical documentation Contribute to continuous improvement of our home care services About You Registered Pharmacist with AHPRA Accredited with the Medication Management Review (MMR) CredentialStrong clinical knowledge and a genuine interest in community and aged care Excellent communication and interpersonal skills Ability to work autonomously and manage time effectively A caring and professional manner when working with vulnerable clients Valid driver’s licence and access to a reliable vehicle Why Join Us Supportive and collaborative team environment Opportunity to make a meaningful difference to clients’ wellbeing Flexible working arrangements Travel allowance provided If you are passionate about bringing high-quality pharmacy care directly to clients in their homes, we would be pleased to receive your application. #J-18808-Ljbffr
