Key Details Part time 0.6 EFT (48 hours) to 09/08/2026 Community Development Worker Grade CL 2A (years 5‑10) Based at 549 St Kilda Rd Great staff benefits: 5 weeks annual leave and salary packaging Role Overview The HOPE Suicide Prevention team provides 3 months’ intensive outreach psycho‑social support and brief interventions to individuals discharged from hospital after a suicide attempt or who have presented with serious suicidal ideation. Psychosocial Support Workers work within a multidisciplinary team under clinician supervision, collaborating to promote recovery, minimise risk, and support clients through tailored care plans. Responsibilities Work Monday – Sunday 8.00 – 16.30, including public holidays, with a rotating roster covering approximately one weekend per month. Provide intensive outreach support, develop safety plans, and facilitate community engagement for up to 3 months with each client. Collaborate with clinicians, senior clinicians and leadership within the HOPE team. Screen and assess ongoing risk of suicide, develop personalised suicide prevention plans, and engage clients in strengths‑based approaches. Assist clients in navigating services, provide transport where appropriate, and foster healthy lifestyle changes. Promote wellbeing by connecting clients with community resources, health practitioners and recreational groups. Essential Experience and Qualifications Hold a health‑related qualification and/or be engaged in further studies, OR significant demonstrated work experience in a relevant field. A current Victorian Drivers Licence. Desirable Experience and Qualifications Other relevant qualifications or training underway (e.g., drug and alcohol, psychosocial, or therapeutic interventions). Experience working with individuals in crisis or with mental health issues.
