The opportunity
The Transition Care Program supports older persons as they transition from hospital to achieve their goals to live at home, through the provision of multidisciplinary, community-based care.
In this impactful role as the Transition Care Program (TCP) Coordinator, you will wear many hats - organising client-centred care plans, ensuring seamless coordination and implementation.
Take charge in guiding our clients towards a safe and fulfilling transition home, promoting independence, wellness, and a sense of belonging.
Collaborate with a diverse team of experts, collectively shaping impactful clinical outcomes.
You will ensure every aspect aligns seamlessly with our contractual and manual guidelines.
Key responsibilities
Communication and collaboration across internal and external stakeholders Clinical responsibility and oversight of TCP clients Using extensive clinical and coordination skills to optimise client outcomes Early identification and management of client risk Providing education, support and guidance to staff, clients and families/support person(s) Advocating and educating clients and families to ensure rehabilitation goals are met Drive excellence and continuous improvement in client care Ensure compliance with contract requirements in managing the TCP About you
Current AHPRA registration, showcasing high-level clinical skills and a minimum of 3 years post-graduate Nursing experience In-depth knowledge of the Transition Care Program (TCP) and complex chronic disease management Strong communication skills (verbal, written, and listening), with excellent report writing and interpersonal abilities to liaise with clients, carers, families, and direct care employees Demonstrated capability to provide education, support, and supervision to direct care employees, clients, and their carers Excellent decision-making and problem-solving skills to innovatively manage the provision of quality client care Empathetic and flexible approach to handling the sensitivities of client and employee issues within a clinical environment Current Driver's License Ability to gain valid police and working with children clearances What you will get with RDNS
