About the role: You will be responsible for delivering in-home and community based aged care services that focus on re-enablement, wellness, and increased independence for consumers. Services will align with each individual's Care Plan, developed in consultation with families and key stakeholders. Key Responsiblities: Monitor and report urgent changes in a consumer's health or environment to the Manager Community Services. Maintain accurate and timely documentation, including: Daily journal of visits, tasks and observations Weekly reports and check-ins with the Manager. Encourage consumer and family feedback to continually improve care quality. Uphold strict confidentiality and dignity of all consumers. Work collaboratively with Indigenous Knowledge Centres (IKCs) and the Healthy Lifestyle Program to support participation in social and community activities. The ideal candidate will bring: A genuine passion for working with Elders and supporting community wellbeing. Strong communication and interpersonal skills. Cultural awareness and a respectful understanding of Ailan Kastom and community protocols. Ability to work independently and maintain professional boundaries. Basic administrative and reporting skills. Minimum one (1) year experience in a similar role. National Police Check upon commencement. Working with Children (Blue Card) for paid employment. Certificate III in Aged Care or related discipline or willingness to undertake studies. Current First Aid and CPR (or willingness to obtain). Queensland Driver's Licence. Comprehensive knowledge and understanding of languages specific to the Torres Strait including: Meriam Mer, Kala Lagaw Ya, and Creole would be highly desirable. Ready to Apply?
