We’re looking for a dedicated Health Care Navigator to join our team in Wagga Wagga (Wiradjuri Country) on a part time (30 hours per week) basis In this role you will collaborate with our care coordinators in addressing the diverse social needs that influence health outcomes. You will take a holistic approach, helping individuals overcome barriers related to finances, transportation, housing, legal issues, and social isolation. If you’re passionate about fostering healthier communities and empowering people to lead fulfilling lives, we want you on our team Selection Criteria: The Essentials for Our Health Care Navigator role Qualifications that Shine: You hold recognised credentials in Community Services, Human Services, Social Work, or a Health-related field. You’ve got the knowledge to back up your passion. Advocacy Champion: You’ve got hands‑on experience advocating for and connecting participants with local services (aged care, NDIS, community services) to help them hit their health and wellbeing goals. You’re their go‑to resource Person‑Centred Pro: You know how to put the “person” in person‑centred care You excel at working collaboratively with participants and their families to create tailored support plans. Health Detective: You have a keen understanding of the wider determinants of health—social, economic, and environmental factors—and how they impact communities. You see the big picture Relationship Builder: You’re a networking ninja You thrive on collaboration, forging strong partnerships across diverse organisations to deliver the best outcomes. Communication Pro: Your communication skills are top‑note You connect effortlessly with people from culturally and linguistically diverse backgrounds and First Nations communities.
