About the Opportunity As an aged‑care case manager at Oak Home Care, you will play a vital role in coordinating supports that enable older clients to live independently, safely and meaningfully at home. You will work in a dynamic and client‑focused environment, where satisfaction, service quality and positive outcomes are key to long‑term relationships and organisational growth. Coordinating supports: Informal supports – linking clients with family, friends and community groups. Mainstream supports – collaborating with health, housing and other community sectors. Funded supports – ensuring clients maximise the value of their home‑care package and receive the services they need. Key Responsibilities Work respectfully with all clients, upholding their dignity, privacy and independence. Support clients to understand, implement and maximise their home‑care package funding. Achieve case‑management targets as set by the Aged Care Manager, aligned with organisational goals and the Aged Care Quality Standards. Build and maintain positive, trusting and professional relationships with clients, families and service providers. Meet with potential clients to understand their goals, needs and preferences. Develop high‑quality, individualised and holistic care plans. Conduct quarterly face‑to‑face reviews with existing clients and maintain regular phone contact to ensure service quality and identify changes or additional supports needed. Monitor client budgets and ensure services remain aligned with goals, needs and approved funding. Document all case notes, assessments and care‑related updates accurately and promptly. Qualifications & Experience Required: Diploma of Nursing (minimum) Current police check Current Victorian driver’s licence Experience in client‑facing care roles (aged care or community services) Desirable: Qualifications in aged care, community services or case management At least 6 months experience in aged care or home‑care packages Skills & Knowledge Strong computer literacy (MS Office, client management systems etc.) Ability to work independently and manage competing priorities. Well‑developed analytic, problem‑solving and organisational skills. Experience creating or reviewing care plans or Individual Program Plans. Strong understanding of goal setting and needs‑based planning. Excellent communication skills and ability to work with people of diverse needs and conditions. Knowledge of aged‑care standards, community care practices, home‑care packages and the Support at Home Program. Confidence managing home‑care package budgets and service utilisation. High‑quality writing skills, including reports and care documentation.
