Role details The Home and Community Care (Bowen) at the Bowen Hospital is seeking applications for a Home Support Worker to join their team on a casual basis. For information on how to apply and the mandatory requirements for this role, please refer to the attached Role Description. Your opportunity To provide high quality support services to frail, older people in order to maximise their independence at home and in the community. To be supported by a highly motivated, consumer focused Home Care Coordination Team, liaising closely with Community Allied Health Professionals. About The Team Bowen District Home Care Service is funded by the Commonwealth Home Support Program (CHSP) to provide accessible, flexible and goal orientated basic support services. Our service is also an Approved Aged Care Provider, providing Home Packages to eligible consumers under Client Directed Care. Bowen District Home Care Service provides basic support services, predominately Domestic Assistance and Social Support to approximately 300 consumers within the Bowen / Collinsville district. Potential CHSP consumers are registered and screened by “My Aged Care” and if eligible referred for assessment by a Regional Assessment Service (RAS). Assessment is based on assisting frail aged people, people with a disability and their carers to continue to live in the community as independently as possible. The Bowen District Home Care office is located in the Bowen Hospital Building, Gregory Street Bowen. About Our Hospital And Health Service Mackay Hospital and Health Service has 6 hospitals, 2 multi-purpose health services and 4 community health centres that employ more than 3,700 staff. The district covers an area of approximately 90,000km2 and provides services to patients and their families in Bowen, Cannonvale, Clermont, Collinsville, Dysart, Glenden, Mackay, Middlemount, Moranbah, Proserpine and Sarina.
