Job Description About the Opportunity: This rewarding opportunity will see you join the St Vincent's Private Hospital Toowoomba (SVPHT) as our Bed Day Discharge Coordinator. Join a great team to support quality patient care in a caring, friendly atmosphere. SVPHT is the largest acute private hospital on the Darling Downs with a 197‑bed facility and highly skilled staff. We deliver medical, surgical, maternity, paediatric, emergency, cardiac and rehabilitation services. Competitive hourly rates and approved overtime, paid overtime. Responsibilities Provide a consultative and liaison‑focused service aimed at supporting medical and nursing staff in the assessment, care, management and successful discharge of patients. Collaborate with family and community organisations to ensure smooth and safe transition out of hospital. Ensure effective and timely admission/discharge processes are in place. Case‑manage patients with an extended length of stay. Maintain high quality care and deliver genuine impact. Utilise well‑developed interpersonal skills to support patient and organisational needs. Qualifications Registered with AHPRA (Australian Health Practitioner Regulation Agency) as a Registered Nurse. Previous experience in community and/or facility settings or a similar role. Effective communication skills at all levels. Fully vaccinated against COVID‑19 and recommended to be immunised against vaccine‑preventable diseases.
