Care Partner – Support at Home (HCP Program) Right at Home, Adelaide Region is seeking a qualified, reliable, and passionate Care Partner to join our growing team. Key Responsibilities Provide compassionate support through phone and online enquiries with timely, empathetic communication. Meet with clients in their homes to support assessments and assist in developing tailored care plans aligned with Aged Care Quality Standards. Manage client enquiries, feedback, service adjustments, and complaints with a solution‑focused and person‑centred approach. Assist the Rostering team with master rosters and scheduling tasks to ensure efficient, high‑quality service delivery. Manage daily scheduling changes based on client needs and staff availability. Support the care team through mentoring, training assistance, and performance monitoring. Conduct staff observations and assist in credentialing care workers for relevant duties such as personal care and medication prompting (as appropriate). Maintain accurate client and staff documentation in line with organisational policies and regulatory requirements. Demonstrate adaptability and embrace challenges as opportunities for learning and growth. Participate in a rotating on‑call system for after‑hours, weekend, and public holiday support (if required). Working Hours This role is Monday to Friday, predominantly 8:30am – 4:30pm (flexibility available). What’s in it for you? Permanent full‑time position – no rotating shift work Inclusive, supportive, and committed team culture 12% superannuation + travel reimbursement Growth and development opportunities within a reputable organisation Preferred Skills 1–2 years’ experience as a care worker, care partner, support worker, scheduler, case manager assistant, or similar role in Aged Care or Disability Sound understanding of the Disability and/or Aged Care sectors Strong organisational and time‑management skills Ability to work well under pressure and deliver an excellent standard of care Strong relationship‑building skills with diverse clients Familiarity with Visual Care or similar Client Management Systems Intermediate to advanced Microsoft Office skills (Excel, Word, Outlook) Qualifications & Requirements Certificate IV or V in Disability, Individual Support, or Aged Care (or equivalent) Current National Police Check & Working with Children Check Current First Aid and CPR certificate Current driver’s licence and reliable registered vehicle Full comprehensive vehicle insurance Excellent verbal and written communication skills Smart mobile phone (iPhone or Android) COVID‑19 vaccination Ability to commute or relocate to Prospect, SA 5082 Full Australian working rights (visa sponsorship considered for suitable applicants) Job Summary Join Right at Home Adelaide Central as a Care Partner and make a positive difference every day.
