Overview The Community Specialist Palliative Care Home Team (SPCHT) at Arthur Rank Hospice Charity supports patients with life limiting conditions across Cambridgeshire in a community setting. Supported by the wider multi‑disciplinary team at the Hospice, the SPCHT ensures that our care is holistic and tailored to each individual’s personal situation. Responsibilities Due to retirement, we have an opportunity for someone to join this team as a Clinical Nurse Specialist . We offer a full‑time role of 37.5 hours per week, but we welcome applications from candidates who wish to work part‑time (minimum 15 hours per week). This role will include weekend and bank holiday shifts and may involve flexible hours discussed at interview. Our Clinical Nurse Specialists deliver expert care and support to specialist palliative and end‑of‑life patients and their families/carers. Working closely with other care providers our team will help manage symptoms, support medicine management and facilitate access to other hospice services, such as complementary therapy. The service runs 7 days a week and colleagues will join us on a rota basis. Holistically assess, plan, implement and review therapeutic interventions for patients with complex needs Organise and manage individual caseloads, prioritising and responding to clinical emergencies as required Plan and evaluate programmes of care for patients and carers, undertaking assessments and analysis of physical, psychological and social needs Maintain regular communication with other care providers, such as GP’s and District Nurses Collaborate with the community team and the multi‑disciplinary team to effectively communicate patient needs Provide specialist nursing advice, support and education to other health care professionals and volunteers, and deputise for the Matron when necessary Additional requirements: Registered General Nurse with a post‑registration qualification in cancer/palliative care or long‑term conditions (or a willingness to work toward this) Experience delivering specialist palliative care in a multi‑professional environment Experience of teaching and education Valid driving licence and access to a vehicle for work purposes Subject to an Enhanced DBS Check (Child and Adult Barred lists) Interviews will be held on 19th November 2025 Salary is based on full‑time hours and will be pro‑rated for part‑time posts.
