About the Position Lumia Care is inviting applications for a Care Partner (Administrator) to look after our community care CHSP portfolio who will operate in a self-directed manner to coordinate the in-home care for older Australians who have Commonwealth Home Support funding. Key to the role is case coordination with Support At Home Clients and internal services. We require an outstanding communicator who works clearly, concisely and with full transparency with their participants and their families. Key Experience Required CHSP experience is a must - we are looking for someone who is knowledgeable navigating the CHSP landscape and has previous experience working with this cohort of customers. Being the first point of contact when receiving enquiries from consumers, families and service providers. Providing high-quality administrative support in a fast paced environment via telephone, emails, mail-outs etc. Experience in receiving and reporting consumer feedback and handling complaints. Experience in updating participants of key changes. Monitoring and reviewing participant needs and updating Help Plans. Liaising with Care Partners in the onboarding of prospective new participants. Familiarity with using the My Aged Care Portal. Handling and involvement with SIR’s (Serious Incident Reports). Our Successful Candidate Will Possess Demonstrated experience in a similar role in the CHSP space - intake & ongoing case management. Outstanding communication skills, both written and verbal. Ability to work to competing deadlines and ability to prioritize tasks. Hybrid work environment – work from home as well as in our Hawthorn Office.
