Overview Join to apply for the Nurse Practitioner Primary Health role at West Moreton Hospital and Health Service About the Role The Nurse Practitioner, within credentialed scope of practice, provides nursing leadership, clinical governance, case management and complex clinical care and support for patients within the correctional environment. The majority of health care provided to patients in the custodial setting is based within a model of primary health care and predominantly relates to the management of chronic disease, infectious disease, comorbidity (Alcohol and Drug Addiction and Mental Health Service) and the provision of acute care as required. The role includes the key coordination functions of complex evidence-based care delivery across a continuum and education provision for patients and staff. About You Demonstrated contemporary advanced practice, expert clinical skills and knowledge in credentialed scope of practice and leadership capability enabling the delivery and support of high quality patient focused care. Demonstrates the ability to apply specialised knowledge of contemporary advanced nursing practice and theory in credentialed scope of practice and to participate directly and indirectly in the delivery of clinical care. Demonstrates the ability to build and maintain strong relationships, open communication and a positive environment within and across the multidisciplinary team, actively contributing to the learning and development of others to a high standard. Demonstrates the ability to educate, inform, counsel and support patients and families with contemporary clinical information at an advanced level relevant to patient's care and understanding. Applies knowledge of quality improvement frameworks and associated quality processes including the integration of contemporary nursing research to all clinical activities. Demonstrated ability to interact professionally and maintain professional relationships with all members of the healthcare team both internally and externally to the health service. Mandatory Requirements Appointment to this position requires registration with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse with Endorsement as a Nurse Practitioner and a current annual practising certificate are mandatory. Ability to meet credentialing requirements of WMH is mandatory for this position.
