Practice Support Worker – Foster & Kinship team in Ipswich (permanent part‑time, 3 days per week) A Practice Support Worker collaborates with the case management team to support and maximize the effectiveness of support plans, ensuring planned and permanent goals for children or young people (CYP) are achieved. Responsibilities Actively support the implementation of plans by providing direct support to the child or young person and their families and/or carers. Undertake direct support functions such as facilitating plan goals, supporting contact, transportation, behavioural observation and recording, and providing in‑home and in‑community support to CYP, carers, and permanent plan stakeholders. Facilitate and coordinate available community supports. Support the implementation of client feedback and monitoring systems, aiding their effective use and review. Provide training, coaching, mentoring, and other support for implementing strategies and plans to bring about behavioural change in children, parents, and carers. Contribute to the monitoring and review of assessments and interventions for effectiveness and validity. Assist with event planning and coordination with the Foster and Equipping Teams to ensure adequate service delivery. Complete and regularly update session plans outlining support provided to CYP and households. Record case notes, observations, and implemented supports. About you Demonstrated knowledge and experience working alongside young people in residential and/or youth programs. A relevant qualification or diploma in Human Services, community development, or related fields (desirable but not essential). Experience developing and building positive rapport with a range of stakeholders, including community members, young people, carers, and fellow staff. Experience working with Indigenous communities and culturally diverse people and communities. Understanding or direct experience in child protection or a related industry. Knowledge of complex trauma, neglect, and factors contributing to out‑of‑home placements.
