About Your New Role You will be assisting customers with complex needs in Ballarat and surrounds. You will be at the forefront of delivering exceptional care and executing strategies to support customers reach their full potential. About you You will bring experience providing supports to people with disability including those with Acquired Brain Injury, Mental Ill Health and a complex history including homelessness, interaction with the justice system and limited informal support systems. You are motivated to support people to achieve their goals, including informing and implementing Support Plans, delivery of person centred active support, and facilitating connection with local communities. You will have confidence in the delivery of support within complex environments, including In-Home Support, Supported Independent Living, Clinical Settings, and Community Access. You are flexible and can work across various shifts, including mornings, afternoons, nights, overnights and awakes.
