You will work as part of our team of experienced Health and Adult Social Care Occupational Therapists, Assistant Practitioners and Assistants to assess and treat adults with a range of conditions within their own homes. You will be supported by the wider North Occupational Therapy Team and will work under the direction and supervision of a qualified Occupational Therapist. You will be expected to work under your own initiative to manage a non-complex caseload of community patients appropriate to the role. The role is 30 hours per week; we may be able to offer some flexibility in line with the requirements of the service. You will be required to travel independently and efficiently over a large geographical area to meet the requirements of the role. Main duties of the job As an OT Assistant Practitioner under the supervision of an Occupational Therapist you will be expected to work on your own and as part of a team focusing on excellent quality patient care, including formulating treatment goals and identifying rehabilitation plans. As part of this role, you will be expected to: Demonstrate excellent interpersonal and communication skills, show initiative and team spirit as well as committed to be patient care. Be flexible and able to work in various locations in North Staffordshire, as the service requires. Understand assessment and rehabilitation appropriate to the role. Have good IT skills, knowledge and understanding of criteria for equipment provision. We are looking for enthusiastic individuals with: Keen interest & / or experience of working in rehabilitation services and can demonstrate an understanding of therapy and service needs. Ability to assess new service user referrals and care/support needs. Proactive in developing sound clinical skills in the assessment and management of patients in the rehabilitation & / or community environment appropriate to the role. To deliver a high standard of intervention, whilst managing the demands of unplanned aspects of the role. To take accountability for allocated assessments, planning & implementation of interventions within their caseload. Experience of when to elevate to a qualified team member. Person Specification Qualifications & Training Relevant Level 3 (or above) qualification in healthcare, social care, or education Numeracy, Literacy and ITQ level 2 (or equivalent) Care Certificate is completed in all 15 areas within 12 weeks (pro rata) of commencement of contract. Practitioner level 5 Foundation Degree or equivalent in related subject, or willing to work towards Experience Relevant placements/experience to the role including supporting or contributing to assessment and management of individuals with compromised functional ability Ability to manage time and caseload effectively Working in a team Experience of working in Occupational Therapy Supporting service users, families and carers in dealing with the impact of disability Working in a multidisciplinary team Skills, Knowledge & Abilities Understanding of Information Governance, Data Protection, Care Certificate HCPC, COT standards and Safeguarding Vulnerable Adults An awareness of conditions and disease management and their impact on individuals and their carers The necessary competences required to undertake this role will be provided (once ratified) as a separate document on appointment. They are derived from skills for health and aligned to national occupational standards. An element of these are: basic IT skills, facilitate the individual's management of their condition and treatment plan, contribute to the discharge arrangements into the care of other services, empower families, carers and others to support individuals, facilitate the individual's management of their condition and treatment plan Personal Attributes Working in community settings and have the ability to travel within a geographical area to meet the requirements of the post.
