About this opportunity As a Palliative Care Nurse Practitioner with Dovida, you will play a central role in implementing our Three-Tier Palliative Care Model during a twelve month pilot program. You will provide advanced clinical assessment, symptom management, care planning, education, and collaborative leadership to strengthen palliative care capability across home care teams. This role will be primarily based across three Dovida offices in Melbourne: Blackburn, Kew & Coburg. It will be offered on an initial 12 month fixed term contract and can either be full time or 0.8 FTE depending on the applicant. What’s in it for you? Diagnose and manage symptoms using independent NP prescribing and clinical decision-making authority. Lead complex case management and conduct home visits and telehealth reviews as required. Deliver targeted training to caregivers, nurses, and office staff across the pilot sites as required. Collect and review pilot data, including PACOP outcomes, training participation, referrals, and symptom trajectories. Support implementation of the Three-Tier Palliative Care Model. About you: Endorsed Nurse Practitioner with current AHPRA registration Postgraduate qualification in Palliative Care Nursing or equivalent experience Experience in specialist palliative care (community preferred) Demonstrated advanced assessment prescribing, and symptom management skills Deep understanding of the palliative care approach, end-of-life principles and the Aged Care Quality Standards Full driver's licence and able to travel between Dovida offices in Melbourne as well as to client's homes How do I apply?
