You will support Aboriginalchildren and young people in out of home care to access timely, culturally safe,health care and wellbeing support This roleis about more than coordination — it’s about wrap around care, advocacy,cultural connection and walking alongside children, families and carers In thisrole, you will: Identify and respond tounmet health and wellbeing needs of Aboriginal children in care Coordinate culturallysafe health assessments, including Aboriginal health checks and screeningprograms Develop and reviewculturally appropriate care plans alongside children, families and carers Support families to accessservices and navigate complex care systems Liaise with ChildProtection, carers and service partners Maintain accurateclient and program documentation We arelooking for someone who brings both professional capability and heart to theirwork. You will have: A relevantqualification and registration (RN/EN, Social Worker, Aboriginal HealthPractitioner or similar) An understanding ofclinical and holistic care services and processes. Experience or knowledgeof child protection and/or out of home care The ability to developclient-centred care plans and coordinate service pathways Strongrelationship-building, advocacy and communication skills Cultural awareness anda strengths-based approach to care Why Join AWAHS?
