Clinical Nurse Consultant – Integrated Chronic Care for Aboriginal People Program – Targeted Position type: Permanent Part Time. Position Classification: Clinical Nurse Consultant Grade 2. Remuneration: $68.50 – $69.87 per hour + Super + Salary Packaging. Hours per week: 16. Location: Wonnarua Country – Cessnock District Hospital. Applications close: Sunday 25 January 2026. Targeted to Aboriginal or Torres Strait Islander people under section 21 of the Anti‑Discrimination Act 1977. Aboriginal or Torres Strait Islander applicants must demonstrate status and address selection criteria. Where you’ll be working You’ll be based on Wonnarua Country at Cessnock District Hospital, working in a shared office with a close‑knit, family‑like team. You’ll collaborate with an Aboriginal Health Practitioner, ICAPP CNC and a chronic disease RN. What the role looks like Support Aboriginal people across Cessnock, Muswellbrook, Singleton and Maitland. Provide clinical assessments, case management, care coordination and collaborate with local GPs and primary health networks.
