We are seeking Care Coordinator who is motivated, reliable, and passionate about aged care. Delivering services under a range of funding sources including private pay clients, My Aged Care and NDIS, the primary responsibility is to provide quality in home care and assistance to our fast-growing client base. The successful applicant will be working with our care team, liaising with Clients, Family Members, Staff and a wide range of Stakeholders to ensure services are provided in accordance with client's needs, goals and preferences. Assessment, planning, and reviewing care plans as required, services are Client focused and Consumer always directed with quality outcomes achieved using a flexible, responsive, creative and collaborative approach. In addition to case management, this role will manage inside and outside sales enquiries, develop and maintain new and existing referral sources. Qualifications The Care Coordinator will be responsible for: Building strong connections with clients and their families Assigning staff to clients based on staff qualifications and availability, as well as client preferences Organise support to clients with disabilities and monitoring and reporting on their progress Referring clients to agencies that can provide additional help Assessing client needs and resources for health, welfare and other facilities and services Responding promptly to emails, enquiries and requests from clients, staff and management Maintaining accurate records of incidents and feedback as required Re-organising work priorities to meet the needs of a fast-paced environment Supporting all clients to meet their goals Conducting Client Care Assessments and Care Planning Supervision and management of Care Staff Discussing and promoting Right at Homes services with medical professions and referral sources Reporting to head office Ability to work in a fast-paced environment Preferred Skills Experienced in assessing and planning care needs Sound understanding of the Aged Care and Disability systems A strong client focus which is based on their goals, preferences and needs Demonstrated experience managing staff Current National Police Check Working with Children Check Full Drivers Licence All candidates must have the right to work in Australia Organisational, administrative and time management skills #J-18808-Ljbffr
