We have an exciting opportunity for an experienced Director - Primary Care to join our future-focused and rewarding organisation. Reporting to the Executive Director - Primary Care and Integration, this role is an important member of the Senior Leadership Team.

Darling Downs and West Moreton PHN (PHN) is a not-for-profit organisation that works at a regional level to achieve improved whole of system health care.

Our key objectives are to:

  • increase the efficiency and effectiveness of health services for patients—particularly those at risk of poor health outcomes, and
  • improve coordination of care to ensure patients receive the right care, in the right place, at the right time.

Our PHN achieves this by working with local health care stakeholders and community to:

  • fund programs and services based on local health needs;
  • support primary health care professionals, like GPs, general practice staff, and allied health professionals, to improve the care they provide to their patients; and
  • support the development of a local sustainable health care workforce.

About the role:

As part of the senior leadership team, you will contribute to the development and implementation of the PHN's strategic framework, performance accountability mechanisms, and organisational capability development to ensure the organisation delivers on its strategic directives and meets the Department of Health and Aged Care (DoHAC) objectives set for PHNs.

  • Lead the primary care team to enable general practices and allied health to deliver high quality and accessible care through support, resourcing, and driving performance.
  • Lead the effective planning, design, implementation, and monitoring and evaluation of primary care transformation and system integration initiatives, ensuring activities are aligned with the organisation's strategic plan, are evidence-based, meet population health needs, and address service gaps and access issues.

Key purpose of the role:

  • Provide strategic and operational leadership to enable primary care transformation including needs assessment, priority setting, and innovative solutions to workforce, service integration and coordination, quality improvement, and emergency preparedness.
  • Lead the ongoing development and implementation of the Healthy Primary Care plan and deliver on primary care transformation and health system integration initiatives.
  • Lead the operations of the primary care team, providing direction, development, and coaching to achieve a high functioning team.
  • Work collaboratively with a wide range of stakeholders to identify service gaps and access issues and implement evidence-based solutions to address these in collaboration with health system partners, fostering integration at the system level.
  • Build and maintain relationships with key stakeholders including GPs, Allied Health, HHS, government bodies, national organisations, and local services.
  • Provide advice and thought leadership to inform the development and implementation of innovative evidence-based initiatives in collaboration with stakeholders.
  • Manage and monitor program reporting, human resources, budget, and service risks/issues across the Primary Care Team.
  • Lead the design and implementation of relevant business strategies, plans, and procedures.
  • Function as an effective member of the Senior Leadership Team, and work collaboratively across the organisation to support its strategic objectives.

Selection Criteria:

  • Extensive experience in leading complex and long-term change in the health or community setting.
  • Exceptional people leadership and operational management skills.
  • Relevant tertiary qualifications and experience in health, social services, and/or health management and planning.
  • High level knowledge and understanding of contemporary primary care models and reform opportunities.
  • Demonstrated systems thinking approach supported by strong analytical and problem-solving skills.
  • Ability to nurture strong working relationships and drive change processes with internal and external stakeholders including government, non-government, and primary care sectors.
  • Highly developed written and verbal communication and interpersonal skills.
  • Ability to apply program and project management tools, processes, and methodologies.
