Home Support Assessor

As a Home Support Assessor, supporting clients in the Barwon South Western region, you will conduct face-to-face home support assessments with older Australians to enable access to the care they need to remain living independently in their chosen community. You will use a standardised national assessment process that focuses on wellness.
Working closely with the client, you will develop support plans with individualised goals. You will match and refer older adults to services which can assist them to achieve optimal wellness and healthy ageing.
