About Us:
YNA is 100% Australian owned & managed, with operations nationally. We are an approved NDIS home care provider, supporting paediatric clients with high health needs, a preferred supplier to disability service groups, residential aged care groups & a contracted provider to hospitals.
The Opportunity:
YNA is seeking a suitably experienced, passionate & motivated Registered Nurse leader with Paediatric experience to support the YNA clients with high health needs living in the community. This is a full-time role, based from Adelaide.
The purpose of this position is to:
- To case manage a portfolio of direct care paediatric clients;
- Ensure client centred, individualised care, positive client outcomes & enhance the care of clients in the community;
- Coordinate & support the provision of comprehensive multi-disciplinary care involving assessment, prevention, referral & health promotion;
- Be a valued part of the community and clinical nurse team
- Actively attract & retain health & support staff for the YNA team; and
- Establish & maintain professional relationships with staff, clients & their support network.
About You:
- Registered Nurse with minimum five year's relevant experience to build the capability to support complex paediatric clients living in the community
- Exceptional communication skills & attention to detail
- A passion for providing quality health care & community support
- Current AHPRA registration & unrestricted Drivers Licence
- Current Police Clearance, WWCC & other Clearances required
- Ability to coordinate & lead a team (prior leadership experience preferable)
