Looking to join a team coordinating care and services to support patients in their own home?
We have a part-time opportunity available to work within the SA Community Care (SACC) and the My Home Hospital (MHH) program.
Using your clinical skills, you will ensure a comprehensive client-centred management plan is achieved for our clients together with:
Conducting clients' initial phone assessment, care planning and goal settingCoordination of in-home services to support clients receiving care at homeBuilding and maintaining effective relationships with a variety of different stakeholdersMonitoring patient progress and updating care plans as appropriateLiaison with healthcare professionals and specialistsDischarge planning including referrals to long-term servicesProvision of telephone support, education, and adviceOur Case Coordinators work a rotating 7-day roster between the hours of 7am & 8pm.
You will be rostered to work alternate weekends in the comfort of your own home.

What will set you apart from the rest?An experienced clinician, you are customer-focused and driven to deliver best practice outcomes across our programs.
Degree qualified as a RN, it is essential you bring a clinical background in an acute hospital setting.
3-5 years' experience in acute hospital as a Registered Nurse with unrestricted AHPRA registrationRecent experience in care coordination (highly desirable)Strong customer service orientation and relationship management skillsExcellent clinical reasoning skillsAbility to work in a fast-paced environmentProficient computer skills to enable fast and accurate data entry into patient electronic medical recordsOur Vision is to be the healthiest workplace in Australia by 2030.
