Telehealth - Care Coordinator

Are you a caring, kind, and compassionate individual keen to deliver evidence-based interventions?
Our team is seeking an experienced nurse or Allied Health professional (with AHPRA registration) to deliver goal-directed, early intervention tele-health services including care planning, health coaching, and support coordination with a highly engaged patient population.
We are looking for both 12-month fixed term and permanent full-time employees.
You will have the opportunity to work flexibly as the role can be hybrid working arrangements.Cogent Thinking's primary purpose is to lead the way in preventing and managing workplace injuries and illnesses with tailored solutions.
We have learnt that workforce health and wellbeing services that are good for people, are also good for business.Job DescriptionTasks include:Support and care for clients with empathy and expertise to improve their personal health and wellbeing goals.Undertake coordination of supports and wellbeing services for a highly engaged and motivated client population.Provide initial early intervention triage with clients, supporting both physical and mental health services.Undertake relevant case coordination activity including liaison with all stakeholders; provision of email updates; and creation of appropriate documentation such as treatment plans or other documentation as required.QualificationsRequirements/QualificationsExperienced Nurse or Allied Health practitioner with full registration.Mental health support service experience highly desirable.Caring and empathic nature.Experience in care coordination or occupational rehabilitation.Well-developed written and verbal communication skills.Good IT and time management skills.This is a fantastic opportunity for someone seeking to work within a dynamic and growing organisation with a terrific teamwork environment where client focus, team spirit, and respect are key features.
