Who are we?healthAbility, as the name suggests, plays a unique and valuable role in the health system.

Our role is also supporting people to self-manage long-term health conditions, such as diabetes and mental health challenges, with multidisciplinary teams delivering community-based programs in a comfortable setting.
We support people, including older adults and those with disabilities, to stay living at home longer and to live well in their community.
Our broad range of primary health services, care management, and in-home and community support services can be tailored to individual needs, with major hubs in Box Hill and Eltham, allowing for local service delivery in the Eastern and North Eastern suburbs of Melbourne.

Job DescriptionThe CCAG is an engagement forum that enables healthAbility to partner with clients and consumers in the planning, design, delivery, measurement, and evaluation of healthAbility's services.
The Chair of the CCAG is also a Consumer Representative member of the Q&S.
The Q&S ensures high standards of care and safety by monitoring outcomes, guiding best practices, and facilitating continuous improvement.
The Consumer Representative on the Q&S is responsible for championing the consumer voice, representing healthAbility's ongoing partnership with consumers to ensure accurate representation at the highest levels of the organization's governance structure.
