About the role Multiple positions available, full time and part time (0.7 - 0.9) fixed term until 30 June 2025 with possibility to extend.
Hybrid work environment based in Perth.
Reporting directly to the Team Leader Client Services, you will be responsible for:
Facilitating individual and group information and education sessions, to people living with dementia, those with cognitive concerns and/or, their families and carers.Providing specialist and intensive assistance to help individuals impacted by dementia to understand and access aged care and connect with other relevant supports in the community, including My Aged Care.Ensuring services are delivered from an enablement perspective, empowering clients to take an active role in directing their own care and services.Building relationships with regional stakeholders/key referrers and leveraging internal and external resources to develop responses to consumer needs to address barriers to accessing services.Developing and embedding referral pathways so individuals are referred to the most appropriate service for their needs.Some regional travel involved (once or twice per month) with the occasional overnight.What is in it for you? Salary benefits: We offer all employees the opportunity to salary package up to $15,900 each year for living expenses and up to $2,650 each year for entertainment benefits.
Leave allowances: In addition to an annual leave allowance of 20 days (pro-rated if part time), you will also receive three days paid end of year closure leave, one day paid birthday leave, up to six weeks parental leave and 17.5% annual leave loading.
Work/life balance: Flexible, hybrid work arrangements - option to work from home 2 days per week.

About you To be successful in this role, we are looking for someone with the following:
Formal qualification in a related field of either Social Work, Allied Health, Nursing and/or significant experience working with clients in the delivery of a community-based service or within a health-related field.Experience and/or knowledge and understanding of the impacts of dementia for individuals, carers and their support networks.Experience and knowledge of group facilitation and group dynamics.Embraces new ideas and change initiatives.Achieves results through a commitment to quality, planning and efficient resourcing.Builds, fosters and sustains relationships (internally and externally).Continually develops and applies specialist and technical skills and expertise to achieve work outcomes.Communicates clearly and openly via verbal and written channels across different audiences.Valid driver's licence as this role requires frequent intra-state travel.Our values Diversity and EqualityRespect and InclusivenessIntegrity and AccountabilityAbout us Dementia Australia is the source of trusted information, education and services for the estimated half a million Australians living with dementia, and the almost 1.6 million people involved in their care.
