Empowering individuals to achieve their personal goals and enhance their quality of lifeProviding crucial support to families, alleviating caregiving pressuresEnsuring efficient use of NDIS resources to maximize benefits for individualsKey Responsibilities As a Support Facilitator/Case Manager at RCS, you'll be at the forefront of providing comprehensive support to people with disability.
Your key responsibilities will include:
Engaging in evidence-based practice for case formulation, assessment, reporting, and interventionsManaging a long-term caseload, meeting KPIs and ensuring quality careMaking strategic referrals to allied health professionals and service providersBuilding and maintaining productive relationships with individuals, stakeholders, and service providersExpertly managing individuals NDIS plans within allocated budgetsDeveloping and providing timely reports on outcomes and success indicatorsCreating tailored, person-centered strategies aligned with funding guidelinesGuiding individuals through NDIS processes and accessing community supportsMaintaining accurate and confidential records in compliance with privacy standardsProviding excellent customer service and direct support to individuals and their familiesAdvocating effectively for individuals as part of a multidisciplinary teamCompleting implementation, progress, and annual reports for individualsCoordinating support services in accordance with NDIS funds and budgetsQualifications and Experience The ideal candidate will possess:
Minimum 3 years of experience in disability services or case managementDegree-level qualification in a relevant field (social work, psychology, allied health, nursing, or community services)In-depth knowledge of the NDIS framework, Supported Independent Living (SIL), Specialist Disability Accommodation (SDA), Assisted Daily Living (ADL) and Community Access (CAS)Experience working with individuals with complex disabilities and high-intensity care requirementsStrong leadership, communication, and relationship-building skillsExceptional organizational abilities and time management skillsA compassionate approach coupled with a commitment to professional standardsHighly developed verbal and written communication skillsComfort with technology and ability to work both autonomously and as part of a teamBlue card and NDIS Workers Screening Clearance (or willingness to obtain)Current Australian Driver's LicenseWhat We Offer At RCS, we value your contribution and offer:
