We provide services and supports to adults, and families with accompanying children in the form of accommodation, case management, assertive outreach, financial assistance, community connection and referral services.
About the roleWe are seeking a values-driven and proactive Case Manager You will provide case management support to assist people to achieve housing stability.Reporting to the Team Leader, this is a fixed-term part-time position, 9 days a fortnight, contracted for one year, based in Stafford, QLD.
Salary and conditions are in accordance with Social Community Home and Disability Services Award Crisis Level 2.
You will successfullyEstablish effective relationships with clients that encourage self-determination and participationProvide case management support to individuals in accordance with TSA case management models and standards of practiceComplete thorough assessments, inclusive of needs, risks and vulnerabilitiesConduct ongoing assessment of the individuals' presenting issues, vulnerabilities and needs from initial intake to exitDevelop individual case plans in collaboration with clients that include a range of interventions addressing the needs or goals of clientsImplement case plans with active client participationAssist clients to access support services and establish community supportsDisplay high levels of emotional intelligence, with an awareness of how personal values and beliefs can impact workParticipate in regular supervision and case review meetingsYou will haveTertiary qualification in Community Services, Social Work or related field.
Degree level qualification preferredRelevant experience in a social service environment including in client assessment and case managementA QLD Blue Card (Working with Children's Check)A current QLD Driver's LicenceWhat we offerAs a registered NFP we offer our eligible employees real and meaningful benefits such as:
