We have a highly engaged, diverse and collaborative workforce that is unified by our goal to help people live healthy and fulfilled lives.About HIP:The Health Independence Program (HIP) is made up of a number of programs including community rehabilitation, specialist services and clinics, and care management.The Fast Track Care Management Team is a highly capable and energetic multidisciplinary team of nursing and allied health professionals with support from geriatricians that provides short term care management for vulnerable clients in the community. We are always client-centred and work as a team.We offer an exciting opportunity for an Occupational Therapist to work with a great bunch of people supporting a diverse range of clients. The Occupational Therapist provides important advice, clinical assessment and support for our clients so they are able to remain safely at home.About the role:This position is part time (3 days/24 hours a week) fixed term until 31/12/2024 and is a great opportunity to try a new role with a new team where you will make a real difference to the lives of our clients.As the Fast Track Occupational Therapist you will:be based mainly at the Repatriation campus of Austin Hospital, but will collaborate across Austin Healthwork with clients and families in their homeswork with clients and families in collaboration with the care managers to provide a safe home environment for clientsprovide advice about occupational therapy across the fast track and chronic and complex client teamsbe supported by the broader HIP community to enhance your own learning and developmentA regular day will see you:applying your skills, knowledge and creative thinking to develop client centred care plans and working with clients to achieve their goalsworking both on campus and in the communityhaving fun working with an experienced multidisciplinary teamKey responsibilities:client-centred assessment, care planning and client management of community-based clientsprovide reports, ongoing referrals and recommendations for longer term management, equipment and home modificationscontribute positively to the multidisciplinary team environmentAbout you:You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care.
