Join our NDIS Support Coordinator team and support individuals and families in implementing their NDIS plans.
The Support Coordination team connects people with a National Disability Insurance Scheme (NDIS) Plan to access a range of supports that suit their individual needs.
As a Support Coordinator, you will provide varying levels of coordination and transition support for ages 0 to 65 years old, considering individual circumstances and complexity.
Participants will be matched with community-based and/or mainstream programs to fulfil their individual needs while receiving continuous support and assistance.Benefits of working for usAttractive salary and eligible for salary sacrifice options - SCHADS Award level 4Working within a hybrid flexible modelFlexible working hours and 9-day fortnightWell established team with a great cultureWorking within a multidisciplinary team within Fortify Health Foundation, Fortify Health Group and Fortify Health KidsTo be successful you must possessA diploma or tertiary qualification in Human Services, Social Work or related disciplinesDemonstrated strong time management skills including prioritisation of competing tasksDemonstrated experience in the provision of support and coordination for people with a lived experience of mental health and other disabilitiesExperience working with culturally and linguistically diverse personsIn-depth understanding of the NDIS and experience working within the NDIS sector for a minimum of two yearsHigh level of interpersonal and communication skills to build rapport and interact effectively with clients, staff, and other agenciesExperience in preparing detailed reports as per NDIS requirementsComputer literacy, word processing, and database competenciesCurrent Queensland driver's licenseA Paid Blue card or the ability to obtain one is essentialNDIS worker screening check or the ability to obtain one is essentialRights to work in AustraliaTo ApplyIf excited by the information above, we look forward to receiving your application, including your resume and cover letter expressing your interest and suitability for the role.For further information about the role, please contact Type#J-18808-Ljbffr
