About the role We are seeking a highly motivated Care Consultant to join our team in the Northern Metro suburbs of Melbourne.
As a Care Consultant, you will spend 70-80% of your day visiting clients, assessing the ageing-related needs of the customers and facilitating necessary supports and services to ensure that customers can live safely and independently in their own homes.
This role is responsible for the following:
Intake, initial and ongoing assessmentsInitial and ongoing service planningCase management and goal-directed care planningRegular/ongoing home visits and monitoringAbout you To be successful in this role, you will need the following:
Qualification in Case Management, orUndergraduate qualification in Nursing / Allied Health / Social Work is essentialDiv 1 Nursing Qualification - desirable but not essentialPrior case management experience will be highly regardedExperience completing initial and annual customer assessments and care planningKnowledge of the Home Care Package industryMust be willing to undertake a police checkFor You Baptcare is focused on living our values, not just talking about them.
