At Choices Australia Support Services, we not only support each participants' journeys but also nurture the personal and professional journeys of our team members, making our workplace a source of both personal fulfilment and professional achievement.About the Role We are seeking a committed and empathetic NDIS Support Coordinator/Psychosocial Recovery Coach for a casual position.
This role is critical in providing tailored support to NDIS and other funded participants, focusing on those with intricate psychosocial challenges.
The successful candidate will excel at building strong relationships with government stakeholders and participants, understanding their unique needs, and skillfully navigating the NDIS system to help them achieve their individual goals.
Here's a snapshot of your day Establish and maintain relationships with government, agency, other stakeholders, and participants, particularly those with complex mental health, psychosocial, and dual diagnoses (AoD).Report directly to the Managing Directors, Chief Administration Officer, and Chief Operations Officer.Conduct thorough assessments to identify nuanced needs and goals.Develop, execute, and oversee individualised support plans, considering complex mental health needs.Assist participants in effectively understanding and using their NDIS plans.Meet with participants in person as required.Link participants with supports, specialised community resources and services.Advocate for the unique needs of participants with complex conditions to service providers and stakeholders.Regularly revise support plans to ensure they remain relevant and effective.Provide specialised crisis intervention and psychosocial support.Maintain precise and timely documentation, adhering to NDIS protocols.Collaborate with mental health professionals and other healthcare providers for integrated support.The Ideal Candidate Tertiary qualifications in Social Work, Psychology, Counselling, or related fields with a focus on mental health (preferable but not essential).Minimum of 2 years of experience in an NDIS Support Coordinator or Psychosocial Recovery Coach role (ESSENTIAL).Experience with complex mental health, psychosocial, and dual diagnoses and providing tailored support.Comprehensive understanding of the NDIS framework, particularly in relation to mental health services.Exceptional communication and interpersonal skills, with the ability to engage with diverse individuals.Strong empathetic and patient approach, with a focus on participant-centred practices.Excellent organisational and time management abilities.Skilled in complex problem-solving and informed decision-making.Proficiency in detailed record-keeping and reporting.Commitment to ongoing professional learning in mental health and disability support.The Essentials Current driver's licence and willingness to travel as needed.First Aid/CPR.Mental Health First Aid (preferred but not essential).NDIS Worker Screening Check.NDIS Worker Orientation Module.Proficient technology skills - NDIS Software & Microsoft 365 (ESSENTIAL)Two professional references (ESSENTIAL)Can we inspire you to join us?
