The Opportunity The role of the Psychosocial Support Service worker is to provide non-clinical psychosocial supports to young people aged 12-25 years of age as well as their families and carers (as appropriate), that are referred to headspace Caboolture to achieve their recovery goals.
headspace Caboolture aims to provide evidence-based, developmentally appropriate and family inclusive care and this role is an important part of the multidisciplinary team at headspace Caboolture.
Some of your key responsibilities will include:
Provide youth friendly, flexible, care coordination and culturally appropriate short term, low intensity support (non-clinical) for young people with mental health presentations using a strength-based, solutions focused approach to assist young people, their family, carers and support networks to work in a collaborative way.
This also includes the provision of psychosocial group programs. Ability to support young people where there may be co-occurring conditions such as intellectual, cognitive, physical disabilities and/or substance use disorders. Ability to support young people to navigate different domains such as access to physical and mental health, substance use, social isolation, family violence, housing and justice system services. Contribute to the assessment of the young person and family's needs, with a particular emphasis on assessing their capacity and strengths. Work collaboratively within a multidisciplinary team to support decision making processes relating to young people and their families, friends and carers including participation in multidisciplinary case meetings. Work from a person centred and family inclusive framework of practice to facilitate the improvement of mental health outcomes for young people and their families. Strengthen shared care models and partnership development with other service providers. Develop and deliver age and developmentally appropriate group work programs that enhance the mental health resilience and social and emotional wellbeing of young people in the local community in collaboration with other headspace Caboolture staff. Hold a minimum Certificate IV Mental Health or other relevant post-secondary qualifications in Health, Disability, Education, Youth Work or Community Development setting. Demonstrated ability to engage meaningfully with young people and their family and friends with a passion and determination to make a difference to the social and emotional wellbeing of young people and their families. Ability to work independently and seek guidance and support, when necessary, as part of a multidisciplinary team. Excellent interpersonal and communication skills (written and verbal) to effectively liaise, build relationships and partnerships, consult and negotiate with a wide variety of stakeholders. Demonstrated experience coordinating and facilitating youth programs, events and activities. A broad understanding of the challenges and experiences of young people in Australia, including specific community factors. Ability to prioritise and manage multiple and competing work tasks and deliver to agreed deadlines. Have some flexibility to work after hours as required. Other Requirements Successful applicants will also need to hold a 'C' class Open driver licence, State Working with Children Check and hold, or be eligible to apply for, a Licensed Care Suitability Check issued by the QLD Department of Child Safety, Youth Justice and Multicultural Affairs.
