Identify and engage Members who may benefit from chronic disease management services through outbound contact processes.
Conduct telephonic healthcare assessments with Members.
Provide quality, safe, appropriate information to members with health/care needs.
Person-centred care planning, according to member goals and immediate health needs; referring people with care needs to other service providers as required; regular review to assess outcomes and modify care plans as appropriate.
What are we looking for?
Accredited Practising dietitian – current unrestricted registration with Dietitian's Australia Computer Literacy – competency in MS Office suite 5+ years (full-time equivalent) experience as an Accredited Dietitian.
