This is an exciting role with a broad caseload.
Whilst you'll have the autonomy to structure your own day to meet key deliverables, you will also be supported by an experienced MDT team.
Together you will deliver patient-centred, evidence-based care in an environment that promotes continuous quality improvement consistent with the values and ethics of HammondCare and the Australian Health Practitioners Regulatory Agency (AHPRA).
The essence of this role Provision of a high-quality Dietetics service to HammondCare Restorative Care team, HCP (Home Care Package) and CHSP (Commonwealth Home Support Program) clients including individual 1:1 home visits and group activities at our various sites Assess and provide intervention to clients in their own homes, carry out rehabilitation home programs to improve functional independence and quality of life Participate as a member of the multidisciplinary team in assessing clients' level of function and care needs, setting care goals, and providing appropriate treatment and interventions Participate as a member of the HammondCare team in the application of the Mission and values to the pursuit of its objectives in a person-centred, continuous quality improvement work environment Participate in quality improvement projects and continuing professional development Provide clinical supervision for students About you Commitment to engage and align with HammondCare's Christian Mission, Motivation and Mission in Action A tertiary qualification as a dietitian acceptable to the Dietitian's Association of Australia Maintain Accredited Practising Dietitian (APD) (or provisional APD) status Significant practical experience or extensive knowledge in a Rehabilitation or Aged Care setting, across inpatient/outpatient or community settings A current driver's licence and safe driving record, with a willingness to travel between HammondCare's sites and services Effective communication and interpersonal skills (written and verbal) Demonstrated organisation and time management skills Demonstrated ability to work independently and as part of a multidisciplinary team Empathy and patience Strong administrative and computer skills Experience working in a multicultural environment An understanding of Work Health and Safety policies and procedures We'll invest in you Our people are the heart of HammondCare, and we support them by providing genuine career opportunities, learning programs and work flexibility, as well as a great range of benefits - fuel discounts, low-cost fitness memberships and more.
