Psychology, Counselling & Social Work (Healthcare & Medical) Full time Fixed Term Part Time Parental Leave Cover 30 September 2024 - 22 May 2025 Position Summary The Grade 1 Social Worker will have consolidated psychosocial assessment and planning skills to deliver clinical services across the inpatient wards and exercises professional judgement to a caseload, adapting routine clinical practice based on valid and reliable evidence.
The successful applicant will support patients and families at Bass Coast Health and use their skills to undertake multi-dimensional, comprehensive psychosocial assessments, apply clinical reasoning, and formulate appropriate complex intervention and discharge plans.
Key Responsibilities Work constructively and co-operatively within a multidisciplinary health setting, the Social Work team and with a wide range of external service providers Engage with patients and their families to provide psychosocial assessment, discharge planning, emotional support, advanced care planning, residential aged care processes, NDIS, and guardianship & administration (VCAT) within agreed timelines Liaise with other multi-disciplinary staff to co-ordinate, facilitate and communicate patient treatment and discharge planning Liaise with external agencies to ensure effective and efficient discharge planning and managed care Maintain adequate records of clinical practice in line with NSQHS guidelines and BCH clinical documentation policy Commitment to continuing professional development Requirements Valid Police Check Working with Children Check NDIS Worker Screen Check Please submit your resume and a cover letter detailing what you can bring to this role and explaining why you are interested in joining our team.
