Rehab@Home is looking for an Allied Health Team Leader to join their team.
The Rehab@Home service is a multi-disciplinary, time-limited community service providing rehabilitation, restorative care, and urgent maintenance level services to clients, including the Transition Care Program within the Metro South at Home.
This position provides operational management and strategic leadership for an interprofessional team of health professionals whilst delivering advanced restorative care and rehabilitation services to stakeholders/consumers within Metro South Health@Home and Queensland Health to achieve excellence in patient care, education, and research.

About the Role Facilitate evidence-based practice in the area of interprofessional team members within Rehab@Home to ensure efficiency and effectiveness of service provision and discharge planning, through implementation and facilitation of quality initiatives, literature reviews, attendance at professional development activities, and involvement in research and education.This includes leading evidence-based practice for own discipline area.Lead and support the delivery of clinical services in accordance with accepted professional practices, standards, and principles, including the legal and ethical requirements, with a high degree of independence.Apply advanced level independent professional judgment regarding the coordination and treatment planning of complex patients as it relates to case management, discharge planning, and service transition.Find out about the role in more detail and how to apply in the attached Role Description.
