BodeWell Community Care | NDIS Support Coordinator
About the Role:
Located at Mill Park State Office - Based at Templestowe Full Time Role - Monday to Friday - 9 am to 5 pm - 76 hours per fortnight Company Vehicle Available for Work Use Laptop, Mobile, Equipment and Uniforms Provided Ongoing Training and Development Opportunities Staff Employee Assistance Program Willing to Use Own Vehicle when Required (km paid) Would you like to be part of something unique?
This role ensures that our participants are supported to live independently in their home by linking them with Directed Care Supports; engaging in the development of assessment and goal planning, creating referrals to appropriate services, ongoing monitoring of budgets and participation in plan reviews, while following Operational Guidelines and NDIS legislation and procedures.

A tertiary qualification e.g.
: Bachelor in Nursing, Allied Health or Social Work / Welfare or Human Services Working with Children Check Clearance NDIS Clearance or a willingness to obtain A valid Driver's License and reliable vehicle Previous experience in NDIS or relevant programs is required Demonstrated experience in managing client budgets Essential to have previous experience working in the aged care and/or the disability field, ideally as a Care Manager / Support Coordinator Experience in assessment and record keeping via databases and/or client management systems, e.g., Carelink + experience would be advantageous Able to work autonomously and as a part of a team Able to manage conflicting priorities – excellent time management skills required Ability to work within Organisational Policies and Procedures and Legislative Requirements Level 3 Support Coordination experience preferred About Us:
BodeWell is an approved provider with the Department of Health and provides government-funded Home Care Packages, Private Aged Care and NDIS Services across Queensland and Victoria.
