What your day will look like:As the Nurse Navigator - Cancer Care and Chronic Disease, you will provide clinical leadership and expertise at an advanced practice level to ensure optimal coordination, clinical management, and care provision for individuals experiencing a life-limiting illness and/or commencing Cancer and Chronic Disease care.
You will work across system boundaries to actively improve health literacy in close partnership with patients, families, and other healthcare providers.
