Under the guidance of the TCP Clinical Manager, you will work collaboratively within a multi-disciplinary team to deliver comprehensive, person-centred care.
You will assess and address the unique needs of each client, providing a range of services that blend preventative, therapeutic, and early intervention strategies.
Your work will contribute to helping clients regain independence, improve their quality of life, and successfully navigate their journey back into the community.In this role, you will respond to referrals, provide expert advice, and guide both TCP coordinators and external service providers on complex discharge planning, risk management, and client safety.
You will also be involved in ongoing quality improvement activities, including the development of policies, frameworks, and educational initiatives.
With a strong focus on clinical practice, you will contribute to screenings, provide essential client education, and actively support decision-making processes.
This role requires a dedicated professional who is accountable for their practice and eager to support both clients and the team to achieve positive outcomes.About YouWe are seeking a dedicated Occupational Therapist with the following key qualifications, experience, and skills:Relevant Qualifications & Registration: Degree or equivalent qualification for Occupational Therapy, along with current registration with the Occupational Therapy Board of Australia.Experience: At least 2 years of experience in rehabilitation, with a focus on managing complex care needs, particularly for older clients in acute or community settings.Skills: Strong communication, negotiation, and organisational skills, with the ability to manage time effectively in a fast-paced, multi-disciplinary environment.Knowledge: A solid understanding of age-friendly principles, occupational health and safety, quality management, and the prescription of specialised rehabilitation equipment.About CALHNThe Central Adelaide Local Health Network includes South Australia's major quaternary facility, the Royal Adelaide Hospital, The Queen Elizabeth Hospital, the Hampstead Rehabilitation Centre, and the Repat Health Precinct.
