About the role: Working in the Northern region of Melbourne, you will be based at our Northcote site.
In this role, you will be an advocate for clients, facilitating discussions to identify our client's goals and developing their care plans.
This is a full-time role working Monday - Friday; alternatively, there is an option of a 4-day working week.
Duties include: Undertaking initial assessments of prospective clients and assisting in making recommendations on capacity.
Assessing, monitoring, reviewing, and reassessing the needs of clients within a holistic framework using a restorative and wellness approach.
Promoting a harmonious team approach by acting as a mentor and supervisor to the Home Care Workers to ensure delivery of community services.
Facilitating effective client pathway support through case discussions and case conferences, reviewing feedback including incidents, hazards, and handovers, and monitoring quality indicators.
Conducting annual reviews with clients, including revising their care plan, goals, services, and budgets to support our clients in living life to the fullest.
About you: You are passionate about what you do and want to make a difference in the lives of our clients, enhancing their self-esteem and empowering them to make choices.
Experience in safety in the Aged Care or Health Sectors is essential.
Experience and expertise in the provision of services in the area of Aged and Community services preferred.
Experience and expertise in working in a Clinical or Allied Health setting.
Case Management experience.
Experience in undertaking assessments, care planning, implementation, evaluation, and documentation of quality client care with a person-centred approach and in line with Consumer Directed Care principles.
The ability to communicate with staff, consumers, service providers, treating health professionals, and other community members to ensure the development of needs-based, consumer-focused services delivery.
A thorough understanding and ability to support compliance with Safety, Equal Opportunity, and other professional and legal requirements which affect day-to-day care and other services for clients.
