Launch Housing South Melbourne has 7 family units for families and children experiencing homelessness.
The Case Management team at Launch Housing South Melbourne is part of Launch Housing's Specialist Child & Family Services area within the broader Families & New Beginnings function.
The Case Management team provides flexible and responsive support to families and children to assist them in achieving dignified and sustainable housing, health and wellbeing outcomes.
The Case Management team provide support to families and children in an outreach capacity, as well as managing support to families in crisis accommodation units located on site.
The Coordinator – Case Management provides effective leadership to the Case Management team at Launch Housing South Melbourne to deliver high quality, housing first and trauma-informed support services to families and children experiencing homelessness using the Launch Housing Practice Framework to guide best practice.
They are also responsible for the management and administrative oversight of crisis accommodation units at Launch Housing South Melbourne and all relevant operational requirements of a Crisis Accommodation site.

What you'll be doing: Ensuring a consistent and best-practice approach to case management, in line with the Launch Housing Practice Framework.Contributing to the delivery of innovative solutions to improve housing and wellbeing outcomes for clients, including children.Advocating on behalf of clients with other agencies to ensure access and delivery of effective services.Supporting and responding to critical incidents of clients and staff, ensuring safety and wellbeing is at the forefront of all decision making.Overseeing the operations of the Crisis Accommodation site and units, which includes advertising, stocking and filling vacant units, liaising with relevant cleaning and maintenance services, and managing client service fees.Developing and maintaining strong relationships with internal and external stakeholders.Assisting in the identification of ongoing and emerging issues in homelessness and housing for families and children and contributing to an environment of continuous quality improvement and innovation.
Supporting a culture and program of continuous improvement by encouraging feedback from stakeholders, staff and clients to inform program and service improvements.Regular review and program development activities for the Case Management team, with a focus on continuous improvement and accreditation requirements.What we're looking for (key selection criteria): Demonstrated understanding of Family Violence and Gender-Based Violence in a homelessness context.Demonstrated experience in client facing human services, social work, or other relevant discipline, with a preference for an understanding of housing and homelessness policy, context, and systems.Program or service coordination/management experience within the community and/or public sectors.Demonstrated ability to work collaboratively with others towards effective client outcomes.Demonstrated experience in overseeing the management and operations of accommodation sites, including crisis accommodations, transitional housing or refuges (desirable).Sound people leadership skills, experience in leading/coaching staff and striving for consistency in performance and behaviour.Have a relevant tertiary qualification (desirable).A valid Victorian driver's licence (mandatory).Demonstrated commitment to the values of Launch Housing and our mission to end homelessness.$55.46 - $55.46 an hour
