As part of an inter-professional telehealth / outreach service delivery team, co-ordinate, assist and deliver high level, evidence-based, tertiary clinical social work services to young people with life-limiting conditions across the continuum of care within the paediatric palliative care service.Work with service partners both internal and external to ensure that young people with life-limiting conditions and their families referred to the service and living outside the south-east corner of Queensland receive optimal and integrated care.Apart from contributing to the development of Australia's best paediatric health service, you will also benefit from a competitive remuneration package and a working environment which embraces professional development, builds capabilities and supports staff to maximise their health and wellbeing.
