Job DescriptionAbout The Role As a key member of the ATSICHS Brisbane team, the Senior Practitioner is expected to effectively assess and manage a caseload of Clients who are referred to the Social Health Commonwealth Psychosocial Support Program (CPSP) and are presenting with complex mental health needs and/or high risk. The Senior Practitioner is responsible for providing leadership and support in the clinical supervision of our CPSP, Social Health Care Coordinators and Home and Healthy Program. Duties Oversee all intake referrals and provide quality support.Ability to maintain data management.Lead the development, implementation and ongoing delivery of the clinical case management practice skills, focusing on improving the behaviours of staff.Case manage staff to provide one-to-one practice improvements to support, ensuring selection of staff are supported based on lowest client outcome performance across branches.Lead the case management and/or case guidance, where appropriate, for high-risk cases requiring multiple immediate responses by different ATSICHS services.Commit to a high level of quality management for ATSICHS through use of LogiQC quality management systems.Provide integration and assisted referrals and coordination between ATSICHS services to facilitate the delivery of an efficient, responsive, and integrated mental health and social needs service to patients.Liaise with other community organisations and government agencies for the purposes of information gathering, referral, networking and promoting ATSICHS Brisbane Services.Provide self-management and team contribution.Work closely with the ATSICHS Brisbane Social Health Manager in relation to the regional development and delivery of CPSP services across Logan and Brisbane regions.Cultivate productive working relationships at all levels, which support the nurturing of internal and external relationships.Culture Enjoy working in an organisation that makes a difference in the community. Base salary of $97,724 per annum (based on qualifications and experience).11.5% Super + access to salary packaging (Increases your take home pay).Mobile Phone provided.COVID-19 Requirement Immunisation history showing you have received two doses of a Covid-19 vaccine is encouraged.Desired Skills and ExperienceRequired Tertiary qualifications in allied health (e.g. social work, psychology or mental health occupational therapy).Minimum of 5 years professional experience in providing support and case coordination to Clients with complex mental health presentations.Demonstrated ability to apply a high level of practice judgement when working with clients in setting and achieving goals.Ability to work with a culturally and socially diverse group of clients, and to apply a culturally responsive framework to guide interventions, with the support of staff and management.Ability to work with diverse people and presentations in providing case managed, recovery-oriented supports.Demonstrated ability in coordinating multiple programs in providing access to holistic care for clients, particularly integrating internal and external supports.Demonstrated knowledge and understanding of the factors contributing to vulnerability, disengagement, and intergenerational trauma of client group.Experience with case management for high risk and/or crisis clients, with high-level planning and organisational skills.Demonstrated ability to work with a minimum of direction and to effectively communicate with and work collaboratively as part of a multi-disciplinary team, and to contribute to the development of creative and innovative services.Demonstrated ability to record accurate and consistent case notes.Demonstrated IT capability - experience in computer-based data management systems and Microsoft Office.A willingness to travel across the ATSICHS Brisbane region of services and work across different sites.A willingness to provide supports and engagement to patients outside of a clinic environment.Working with Children's Card (Blue Card) or be eligible to apply.NDIS Worker Screening Card (Yellow Card) or be eligible to apply.Preferred Current, full registration with the appropriate professional body (e.g. AHPRA, AASW).Experience in delivering supervision or leadership activities.Previous experience in a multi-disciplinary team environment.Previous experience working in an Aboriginal Medical Service.How to apply If this sounds like you, please submit your application by following the links on the recruitment page.
